					<description><![CDATA[Patient F is a 71-year-old black male whose wife passed away two years ago.   He has found someone new in his life. He would like to become sexually active with her but found he can only achieve a partial erection.   Again, this is a very common presentation.   Many times, the significant other has been sick for some time and not sexual; after she passes away, it usually takes a while before the man is interested in another sexual relationship. 					<description><![CDATA[Patient E is a 45-year-old male who state he functions “OK” but he has realized that it takes more stimulation and more time to achieve an erection.  Rarely he was not able to achieve an erection at all.  His erection is not quite as hard as it was a few years ago. He has a beach vacation scheduled with his wife in 2 weeks.   This fellow’s situation is fairly common as well.   He wants something to help him perform better.   Reviewing his history there were no significant medical issues so he was given Tadalafil (Cialis).   His instructions are to take one prior to activity.   On return he was very happy.   He noted there was effect of the meds several hours after his dose.   He said that he felt more like a man than he has in many years!]]></description>

					<description><![CDATA[Patient D is a 58-year-old white male who had his prostate removed 8 months earlier.   His sexual function was normal prior to the surgery, but since the surgery he has no function at all.   This is a common situation with prostate surgery.    This is an anatomic problem.   There are nerves that go to penis that actually run beside the prostate.   Without these nerves being intact, there is no erection.   So, when the surgeon removes the prostate by regular surgery or robotic surgery, it is very easy to damage these nerves.    With robotic surgery there is frequently less injury to these nerves, but it still happens.   Radiation can cause the same problem by damaging the same nerves.   Most urologists will say there are a significant number of people who will regain function after the nerve heals, but not everyone.   This man was easily treated with ICI and he has resumed his normal function.    He is very happy to have his sex life back.]]></description>

					<description><![CDATA[Patient C was a 51-year-old type I diabetic.&#160;&#160;&#160; He cannot remember when his problems began but he hasn’t been able to function sexually in “many years”.&#160;&#160; This fellow has educated himself considerably on the options and was eager to try anything that might work.&#160;&#160; He was evaluated and given a test dose of medication and achieved a workable erection.&#160;&#160; When I walked into the exam room after the test dose, he was smiling from ear to ear!&#160;&#160; Patient C is a very happy man.&#160;&#160; He has regained part of his life that he thought was lost.&#160;&#160; Diabetes, both type I and type II are a significant cause of ED.&#160;&#160; The longer you have diabetes and the poorer the control of the diabetes, the worse the ED.&#160;&#160; Most diabetics have a low testosterone level.&#160;&#160; More on testosterone therapy in a subsequent blog.&#160;&#160;&#160;]]></description>

					<description><![CDATA[Patient B is a 63-year-old male who hasn’t been able to have sex for 5-6 years.&#160;&#160; While we were talking about his situation, I was able to see the pain on his face when he talked about just passing a landmark anniversary and he didn’t even attempt intercourse with his wife.&#160;&#160; As I mentioned with patient A, education is essential.&#160;&#160; This man is obviously depressed and feels like his sex life is over.&#160;&#160; He spent probably an hour talking about his situation and possible treatments.&#160;&#160; Unfortunately, this man was depressed enough that he was not even interested in having a test dose of the medication to see if it would work.&#160;&#160; Patient B is an example of how much pain and suffering is involved in not having the ability to have sex.&#160;&#160; Humans thrive when they are sexual.&#160;&#160; It has many positives for people individually but also helps a couple to feel closer.&#160;&#160; It is very unfortunate Patient B did not even attempt to find a cure.&#160;&#160; &#160;&#160;&#160;]]></description>

					<description><![CDATA[As you recall, I talked about several real patients and their situation in a previous blog.&#160;&#160; Now, I would like talk about their treatment. Patient A is a 55-year-old who began having ED troubles a few years ago.&#160; He was prescribed oral agents by his primary care doctor and these worked initially but over time became ineffective.&#160;&#160; Now he is only rarely able to complete intercourse.&#160;&#160; Patient A is one of the most common presentations of ED.&#160;&#160; One of the most important parts of treatment is to educate the patient that this is a medical condition and not him “being less of a man”.&#160;&#160; Many relationships suffer at this point because the man is afraid of not being able to perform so he avoids sex.&#160;&#160; His partner notices and frequently begins to think he doesn’t love me anymore or he is getting sex elsewhere.&#160;&#160; In this situation, education is extremely important.&#160; Patient A was seen in the office, had an examination and was given a test dose of medication.   He responded well and achieved an erection that was sufficient to complete intercourse.   He was educated in proper use of the medicine and the mechanics of proper application of the meds.   [&#8230;]]]></description>
